Three things happened this week that look unrelated at first glance.

AI and the systems around it are not maturing at the same rate. Technology is scaling. Governance, integration, and accountability are catching up slowly, and in some cases not at all.

None of these required breaking the core technology. They moved through the system exactly as it was designed to work.
What this means for leadership teams
The question most organizations are asking is: What can AI do? That's the wrong starting point.
The more useful question is: Are we structured to support what AI requires?
That means being clear on decision ownership when automated processes fail mid-execution. It means having defined fallbacks when AI can't safely proceed. It means understanding how vendors, networks, and workflows are connected, and what happens when those connections are exposed. It means treating your internal logic, your orchestration layer, your deployment architecture, as something worth governing.
These aren't technical problems. They're operational ones. And they don't get solved by adopting better models.
The real risk isn't that AI doesn't work. It's that the systems around it aren't ready for when it does.
